Health Information
Immunizations
Minnesota State Law (statute123.0) requires that all children who are enrolled in a Minnesota public or non-public school must be immunized. Check your child's records eary and have the information completed before school starts. The number of doses of required immunizations is as follows:
Before starting Kindergarten
- DTP (diphtheria, tetanus, pertussis) • 5 doses
- OPV/IPV (polio) • 4 doses
- MMR (mumps, measles, rubella) • 2 doses
- Hepatitis B • 3 doses
- Varicella (chicken pox)** • 1 dose, unless child has already had chicken pox.
** Documentation of varicella vaccine or history of disease is required for kindergarten entrance.
Before starting 7th Grade
- Td booster ( tetanus) • 1 dose
- MMR (mumps, measles, rubella) • 2 doses
- Hepatitis B • 3 doses
- Varicella (chicken pox)** • 1 dose, unless child has already had chicken pox.
IMPORTANT: The hepatitis B vaccine is a series of 3 shots that takes at least 4 months to complete, so it is important to start planning early!

If a parent or quardian has a conscientious objection or if there is a medical reason your child is unable to receive immunizations, the proper exemption, found on the back of the immunization form, must be completed. Please note that the conscientious objection requires a notarization. CHILDREN WILL NOT BE ABLE TO ENTER SCHOOL IN SEPTEMBER UNLESS A COMPLETED IMMUNIZATION RECORD OR THE PROPERLY COMPLETED EXEMPTIONS HAVE BEEN TURNED IN TO THE SCHOOL.
Low cost immunizations are available for those who qualify through the Hennepin County Health Department. Call 612-348-2741 for clinic hours and locations.
Physical Exam
A report of a physical exam, done within the past year by your health care provider, is requested for all Kindergarten, students entering Grade 7 and all new students to OLP. It is important that the school has information about the health of students. If your child has any health problems that may need special care, please inform the school.

Medications
Medication should be given in school only when absolutely necessary. If your child requires medication, discuss a schedule with your health care provider that would allow the doses to be given at home. It it must be given in school, all medication (both prescription and non-prescription) can be given only with the written consent of the parent and the physician. The medication must be provided by the parent and be in a properly labeled bottle from the pharmacy. See Taking Medicine In School for detailed information. Early Childhood Screening
All kindergarteners enrolling in a public school for the first time must complete Early Childhood Screening. The screening is free and includes an immunization review, develolpmental screening, vision and hearing screening and review of a health history. It is not required, but is strongly recommended, for entrance to a non-public school. See flyer.
Dental
It is recommended that children start a regular routine of dental care before five years of age.
